Our Program
Transcend provides free case management services to clients in Mecklenburg, Gaston, Lincoln, Iredell, Union and Cabarrus counties in N.C., and York County, S.C.
At Transcend, we believe in a person-centered approach, which means we work with you to design plans, find resources and structure support that fits your unique identity and needs. We work with you as a partner to document the goals you want to set and then champion you as we move forward to achieve your goals.
The Process
New clients must first participate in an assessment and will then work with our Case Manager to develop a client plan comprising short- and long-term goals, challenges and support needed. Sessions are typically 50-minutes.
Currently, due to COVID-19, Transcend is hosting all case management sessions from the comfort of your home. This is accomplished via our secure, privacy protected, online tele-case management platform called Simple Practice. Clients who prefer tele-case management sessions must ensure that the location from which they choose to log on is private and quiet, and has a stable internet connection.
